Medicare
Dr. Seth Kaufmann, PT, DPT is now IN-NETWORK with MEDICARE beneficiaries! Call and schedule an appointment if you are covered under Medicare Part B services for physical therapy.
Items to bring before your Initial Evaluation (1st visit):
Driver’s license
MEDICARE insurance card
other supplemental/medigap/secondary insurance cards
Copy of Referral/Script from prescribing physician (MUST HAVE name, NPI, address, office number, and fax number from referring physician)
Seth is unable to conduct the initial evaluation without a copy of the referral/script
Non-Medicare
Dr. Seth Kaufmann, PT, DPT is primarily an OUT-OF-NETWORK provider. This means that Seth does not submit directly to insurance companies. Instead, Seth helps the client with submissions for out-of-network reimbursement, by providing ALL clients with the necessary documentation and itemized receipts. Accessing Seth will be easiest when your health plan requires a deductible that needs to be satisfied, in order for the client’s health insurance benefits to start being utilized. These plans are often complimented with HSA and FSA accounts.
